Medical Record Review & Synthesis

Comprehensive Review: An analysis of all relevant medical records, C&P exams, and private physician statements (especially VA Form 21-2680). Medical-Vocational Translation: Translating clinical diagnoses (e.g., severe neuropathy, advanced PTSD, cognitive decline) into specific physical, environmental, and mental/cognitive restrictions.

Activities of Daily Living (ADLs) & IADLs Assessment

ADL Dependency: Detailed evaluation of the veteran’s inability to perform basic ADLs independently (bathing, dressing, toileting, feeding, and transferring). Instrumental ADLs (IADLs): Documentation of restrictions in managing medication, meal preparation, transportation, and financial tasks.

Safety & Supervision: Independent Living Assessment

Assessing the need for a protective environment or regular supervision to prevent injury or wander-risk. For IL Tracks, we detail the specific ecological, physical, or psychological barriers that prevent independent living in the home or community without assistance.

Functional Analysis

Functional analysis translates documented medical limitations into real-world functioning. We assess physical and cognitive capacities against daily demands to produce a clear, reasoned opinion on their ability to perform ADL.
